In order to have your claim considered for payment from a fund established for compensation of injuries or death resulting from an outbreak of Salmonella, you must complete and return the Proof of Claim form and attach the required documentation.

Even if you have previously filed a proof of claim, you must complete and return this proof of claim form and attach the required documentation.
